Acne (aspe vulgaris) is more often called acne. This is a chronic recurrent inflammatory skin disease, manifested by open or closed comedones, papules, pustules, cysts.

The trigger is a genetically determined increase in the number of androgens in the body and/or an increased sensitivity of sebaceous gland cell receptors to testosterone derivatives. As a result of excessive sebum production, hyperkeratosis occurs, which contributes to the formation of a plug at the mouth of the follicles. This leads to an accumulation of keratin masses, excess sebum and various bacteria in the lower part of the follicle. Then, the expansion of the upper part of the hair follicle and the formation of a microcomedon sequentially occurs. The microcomedon containing keratin, sebum and bacteria continues to expand, the follicle wall ruptures, and the inflammatory process begins. In order to clarify the cause of acne, the following examinations may be recommended:
- clinical blood analysis with determination of hemoglobin concentration, number of erythrocytes, leukocytes and platelets, hematocrit and erythrocyte indices (MCV, RDW, MCH, MCHC), leukoformula and ESR (with microscopy of a blood smear in the presence of pathological shifts).
Acne treatment is carried out on an outpatient basis.
The purpose of treatment is to relieve inflammation, reduce the number of microorganisms (R. acnes), reduce the activity of the sebaceous glands by correcting the level of androgens.
During treatment, the doctor takes into account the prevalence of the inflammatory process, the number of elements, the presence of scarring, and the severity of acne.
Topical retinoids, azelaic acid, benzoyl peroxide, and antibacterial drugs are used for external therapy.
Systemic acne treatment may include antibacterial agents, hormonal drugs, androgen receptor blockers, inhibitors of ovarian androgen production, androgen inhibitors of adrenal origin, systemic retinoids.
