Compound
Each film-coated tablet contains 25 mg of sildenafil in the form of sildenafil citrate. Each 25 mg film-coated tablet contains 1.63 mg of lactose monohydrate.
Action
For erectile dysfunction, the drug MAXON ACTIVE is indicated for use in adult men with erectile dysfunction, that is, the inability to achieve or maintain a penile erection sufficient for sexual intercourse. For MAXON ACTIVE to work effectively, sexual stimulation is necessary.
Contraindication
Hypersensitivity to the active substance or to any of the excipients (microcrystalline cellulose, hydroxypropylcellulose, croscarmellose sodium, magnesium stearate, colloidal anhydrous silicon dioxide, hypromellose 6cP, lactose monohydrate, titanium dioxide (E171), macrogol 4000, sodium citrate dihydrate). Due to the effect of sildenafil on metabolic processes involving nitric oxide and cyclic guanosine monophosphate (cGMP), it enhances the hypotensive effect of nitrates. Therefore, the simultaneous use of sildenafil with drugs that release nitric oxide (such as amyl nitrite) or nitrates in any form is contraindicated. Concomitant use of PDE5 inhibitors, including sildenafil, and drugs that induce guanylate cyclase, such as riociguat, is contraindicated as it may lead to symptomatic hypotension. Medicines intended to treat erectile dysfunction, including sildenafil, should not be used by men who do not want sexual activity (for example, patients with severe cardiovascular disease such as unstable angina or severe heart failure). MAXON ACTIVE is contraindicated in patients who have lost vision in one eye due to non-arteritic anterior ischemic optic neuropathy (NAION), regardless of whether it was associated with previous exposure to a PDE5 inhibitor. The safety of sildenafil has not been further studied. Patient groups: Patients with severe liver failure, hypotension (blood pressure <90/50 mmHg), recent stroke or heart attack, and known hereditary retinal degenerative changes such as retinitis pigmentosa (a small proportion of these patients have genetically determined disorders of retinal phosphodiesterase). The use of sildenafil in these patients is contraindicated.
Use
Use in adults. The recommended dose is 25 mg of sildenafil, taken as needed approximately one hour before planned sexual activity. It is not recommended to use the drug more than once a day. If MAXON ACTIVE is taken with food, the onset of action of the drug may be delayed compared to taking the drug on an empty stomach. If your doctor decides to use pharmacological treatment, he may recommend a 50 mg dose of sildenafil taken as needed about an hour before planned sexual activity. Depending on the effectiveness and tolerability of the drug, the doctor may increase the dose to 100 mg or reduce it to 25 mg. Special groups of patients. Elderly patients. In elderly patients (≥ 65 years), no dose adjustment is required. Patients with impaired renal function. The dosage recommendations described in the section “”Use in Adults”” also apply to patients with mild to moderate renal impairment (creatinine clearance 30-80 ml/min). In patients with severe renal impairment (creatinine clearance <30 ml/min), a dose of 25 mg should be considered due to reduced clearance of sildenafil. If the decision to use pharmacological treatment is made by the doctor, depending on the effectiveness and tolerability of the drug, the dose can be gradually increased by the doctor to 50 mg, up to a maximum of 100 mg (if necessary). Patients with impaired liver function. Patients with impaired liver function (eg, cirrhosis) should consider a dose of 25 mg due to reduced clearance of sildenafil. If the decision to use pharmacological treatment is made by the doctor, depending on the effectiveness and tolerability of the drug, the dose can be gradually increased by the doctor to 50 mg, up to a maximum of 100 mg (if necessary). Children and teenagers. MAXON ACTIVE is not recommended for use by persons under 18 years of age. Use in patients taking other medications. If the physician decides to use pharmacological treatment, an initial dose of 25 mg should be considered in patients concomitantly taking CYP3A4 inhibitors. The exception is ritonavir, which is not recommended for use with sildenafil. Oral administration. To reduce the risk of postural hypotension in patients receiving alpha-blockers, such patients should be stabilized before initiating treatment.”Adamed
